- 100% fall protection when exposed to a fall hazard above 6’
- 100% fall protection when working within 6’ of the leading edge and working outside the“HAZARD LINE SYSTEM”.
- 100% fall protected while working on ladders exposed to 6’ fall or more.
- 100% tie – off to proper anchor point in aerial work platforms.
- 100% tie – off in scissor lifts. Must tie – off at mid rail, manufacture point, or alternate way
approved by the safety department.
- 100% tie – off with cable grabs when working on all sloped roofs, beams, and verticals provided
the cable grab is kept as high as possible.
- Full Body Harness with 2 shock-absorbing lanyards attached to the back Dee – ring used for fall
arrest.
- 3/8” aircraft cable to be used for safety cable. Anchorage requires a minimum of 2 forged steel
clamps.
- Temporary Beam Safe posts not to be spaced more than 40’ apart on 4-sided posts and 60’ apart
on 6-sided posts.
- Floor holes to be covered or guarded immediately. Ensure the cover supports twice the maximum load such as equipment, employees, and materials. Secure cover to prevent
displacement, and mark the word “HOLE” or “COVER” for warning.
- All tools and equipment to be inspected before each use. Damaged tools to be tagged “Danger
Do Not Use” and/or removed from the jobsite immediately.
- All fall protection equipment, electrical equipment, rigging equipment, and ladders, must follow
LPR’S color code policy.
- Safety glasses and hardhats to be worn 100% of the time entering or leaving the jobsite; not left
in gang boxes and then put on prior to start work.
- Dark lens safety glasses are not to be worn inside enclosed buildings where lighting is
insufficient, clear or amber lenses are acceptable.
- Cutting goggles worn 100% while using torch. (Not safety glasses)
- Face shield and safety glasses required while grinding or using deck saw.
- Guards required at all times on all grinders over 2” diameter.
- Quick connects/Back-flow valves to be attached to all oxygen and acetylene gauges. MUST
USE FLASHBACK ARRESTORS ALSO.
- Bottles properly secured in torch carts and in storage racks; no tie wire.
- Oxygen and acetylene bottles being stored must be upright, secured, and 20’ minimum apart with
fire extinguisher.
- Fire extinguisher set-up with all torch sets and within 50’ of all hot work.
- Fire extinguishers must have monthly inspections complete and documented on attached tag.
- Fire extinguisher located in all trucks, cranes, forklifts, aerial lifts, scissor lifts, job trailers,
generators, and by all fuel cans/tanks.
- Ladders to be inspected before each use and secured with rope 3’ above the work platform.
- Ropes staged at all ladder access and egress points to hoist and lower tools and materials.
APPROVED CONTAINERS ONLY FOR HOISTING)
- Must have personal fall arrest in place at any ladder access and egress point exposed to a 25’ fall
or more. (I.E. Retractable, vertical life line with cable grabs)
- Competent Person when using scaffolds.
- A ground-fault circuit interrupter (GFCI) is required while using any electrical equipment.
Including portable welders, 4/6/8 packs, and generators.
- All electrical cords and equipment must have grounds in place and inspected for quarterly color
code program.
- Welding leads must be repaired regularly with approved repair material.
- No splices or repairs within 10’ of the electrode and no damaged or missing insulator on the
stinger.
- Tag lines on all overhead loads.
- Inspect all rigging before each use.
- Steel slings with 3 or more broken wires within a rope lay shall be cut up and discarded
immediately.
- All rigging hardware shall be American made or approved through LPR safety department.
- Proper clothing. No torn work boots, ragged clothes, tank top/muscle shirts etc.
- Pitching of tools and materials to upper elevations is prohibited.
- No horseplay.
- BIC lighters or butane lighters will not be permitted on jobsites. (Matches, Zippo style lighters
are acceptable)
- All employees are expected to maintain good housekeeping. All jobsites and work areas must be
organized and clean.
- Illegal drugs, alcohol, firearms, and other paraphernalia are prohibited on site including personal
vehicles, company vehicles, and equipment.
- All LPR employees are responsible for safety.
